The prevention of tailings dam disasters is described as being an ambitious and challenging goal, made more difficult by the competitive, international and cyclical nature of the mining industry. Sixteen years ago ICOLD issued recommendations for change, and although tailings management has improved in some areas, the mining industry still fails to achieve a zero-failure rate. Suzanne Pritchard looks at a recent report by the United Nations Rapid Response Assessment Series, which hopes to encourage action to make zero catastrophic incidents become a reality.
