Tailings dams are complex and unforgiving systems which evolve over a period of years. The recent failure of Mount Polley tailings facility in Canada has brought global attention to the fact that the reliability of tailings dams is dependent on consistent and thorough planning, design, monitoring and effective operation at every level. After an independent inquiry into what has been described as Canada’s largest industrial accident of its kind, the industry is being steered on a path towards zero failure.
